As we enter into the new season in 2017, this is a good opportunity to update the club on any changes in your swimmers emergency contact details or medical declarations.
 
There are two forms for any medical declarations which are available on the website in the downloads section. If it becomes necessary for you inform the club of any medical declarations, the forms should be completed and accompany each other when they are returned to the club secretary. They can be handed to the Club Secretary on Wednesdays at FLC or sent by email to This email address is being protected from spambots. You need JavaScript enabled to view it.
 
Also on the wbsite is the Change of Details form. If it becomes necessary for you to inform the club of any changes to contact emails etc, or emergency contact details, the form can be completed and can be handed to the Club Secretary on Wednesdays at FLC or sent by email to This email address is being protected from spambots. You need JavaScript enabled to view it.
 
The club keeps in contact with all members via Swim Club Manager and we can only reach you with important information if you keep us up to date of the correct email address to use.  It is also the responsibility of swimmers/parents to keep the club informed of any changes and so please do feel free to use these forms at any time.
